Middle School Sport Physicals

MIDDLE SCHOOL SPORT PHYSICALS: Students who plan on participating in Middle or High School Athletic Activities will need to complete a physical for the 2010 - 2011 school year. Physicals will be available starting at 12:00 p.m. here at the Middle School on Friday, May 7th, with the girls and then the boys will follow. Physicals may also be done at your own doctor's office. Dr. Herald's office will facilitate the physicals here at the middle school. Cost for the physical is $15.00. (Make checks payable to Parchment Middle School.) PHYSICAL CARD MUST BE SIGNED BY PARENT PRIOR TO GETTING THE PHYSICAL. THE DOCTOR WILL NOT PERFORM THE PHYSICAL WITHOUT A SIGNED CARD. PHYSICAL CARDS AVAILABLE IN THE OFFICE.
